Look around the common areas as you answer
Are throw rugs removed or secured with non-slip backing?
Are pathways clear of cords, clutter, and low obstacles?
Are night lights installed in hallways and key areas?
Is furniture arranged to allow easy, unobstructed navigation?
Are carpet edges secured and flooring even throughout?

Bathroom
The highest-risk room — answer carefully
Are grab bars installed next to the toilet and inside the shower or tub?
Is there a non-slip mat inside the tub/shower and on the bathroom floor?
Is a shower chair or bench available if needed?
Is a raised toilet seat in place if needed?
Are daily essentials kept within easy reach in the bathroom?

Most nighttime falls happen getting in and out of bed
Is a lamp or light switch reachable from the bed without getting up?
Is a phone or call device accessible from the bed?
Is the bed at a height that allows both feet flat on the floor when seated?
Is there a non-slip rug or mat beside the bed on hard flooring?

Check every staircase and main entry point
Are there sturdy handrails on both sides of all stairs?
Are all steps well-lit and in good repair?
Are step edges marked with bright contrast tape where needed?
Is the entryway clear of shoes, bags, and mats that could cause tripping?

Answer based on what you typically observe
Do they consistently wear well-fitting, non-slip footwear indoors?
Do they rise slowly from bed or chairs before standing?
Do they consistently use their prescribed cane, walker, or wheelchair?
Do they stay well-hydrated throughout the day?
Have their medications been reviewed for fall-risk side effects?
Has their vision been checked within the past year?
Do they ask for help rather than rushing when tired or unsteady?
Do they do any regular balance or strength exercises?

Medical Safeguards

Have they had a fall-risk assessment with their physician?
Are they enrolled in or referred for balance or strength training?
Has a physical therapist evaluated the home for safety?
Do they have a personal emergency response system (e.g. medical alert device)?
Have their vitamin D and calcium levels been checked recently?
Is their full medication list updated at every doctor visit?
